Medication Refills/Relay Health
Please have your pharmacy fax a refill request. Please allow up to 72 hours for processing. Most pharmacies elect to receive prescriptions electronically. Please let them know our office can be contacted through RelayHealth.
Active patients can register on RelayHealth for the following services:
Payments and Fees
Payment is collected in full upon check-in at the time of your visit. We accept cash and credit cards for all patients and checks for established patients only.
Please review our financial agreement page under the new patient tab. Patients are responsible for payment of missed or cancelled appointments with less than 24 hour notice for the full contracted rate of their appointment. Insurance plans do not cover missed or cancelled appointments. You will be required to sign the financial agreement prior to meeting with Dr. Burstein.

If we do not accept your method of insurance, you will be required to Self-Pay for your visit and we will gladly print a claim form for you to submit to your insurance company.
We currently accept the following insurance companies and will submit the claims for you:
After Hours
Our answering service is available for emergencies only. Emergencies do not include medication refills or routine questions and will not be responded to. Please contact our office at 480-905-8755 for instructions to contact this service. Please provide details of the problem and indicate whether you need immediate assistance or if this can wait up to one hour and our answering service will contact the provider on call. Failure to leave detailed information can result in no response.
If you have a private number, please unblock your telephone by pressing *87. You may turn the blocking function back on after talking with the physician or nurse by pressing *77.

- Bypass the pharmacy for refills and send requests directly to the doctor to fill online
- Ask routine administrative questions
